On June 20, 2017, at 11:30 a.m., an employee was assisting a client with a bath in the client's apartment home when the ceiling above him caved in. The employee was struck in the head and injured by falling debris.

An employee was transporting a container across the street to the trash truck when he was struck by a vehicle. He sustained lower back bruising, a left leg fracture, and a laceration to the back of his head.

An employee was in the middle of a roundabout when they were struck by a vehicle, resulting in injuries to their head, neck, back, hips and legs.

On September 10, 2025, an employee was walking up a wooden wheelchair ramp to a back door at a customer's home when she slipped/tripped on a rug and fell into a wall, hitting her shoulder and elbow. She then fell to the ground, resulting in a fractured hip that required hospitalization.

An employee was getting up off a couch as a client was aggressive. The client kicked the employee, causing him to fall over the arm of the couch and land on the floor. The employee was hospitalized with a fractured left hip.

An employee was sitting in a chair when their legs were struck by a golf cart. The employee's left leg was broken and required surgery.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
