An employee was watching an operator load materials with a forklift. The operator tilted the forks slightly, which caused the material to break its straps and roll off the forks. The employee jumped to get out of the way and fell on the cement floor, fracturing his hip.
An employee was running a drill press. The press caught the sleeve of the employee's welding jacket and pulled in the employee's arm. The employee's arm was fractured.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.
