An employee performing thermoplastic handwork at an intersection was struck by a vehicle in his work zone resulting in a broken leg that required surgery and head injuries that required staples.

Employees were performing roadway painting activities when three employees were struck by a motor vehicle. One employee was hospitalized with a concussion, scratches, and bruising.

On April 22, 2024, a highway construction employee was working on pavement markings on an interstate when a passing vehicle struck them, resulting in a fractured leg and hospitalization.

An employee was operating a truck behind a vehicle laying down road striping. A tractor-trailer veered into the work zone, striking the employee's truck. The employee suffered back, neck, hip, and head injuries and was hospitalized.

An employee was part of a crew that was placing stop bars, turn arrows, and crosswalks at an intersection. A vehicle entered the work zone and struck the thermoplastic handliner, which the crew was using to apply the roadway markings. The handliner is a small hand machine that uses molten thermoplastic (430 degrees F) to produce the markings. The impact of the vehicle on the handliner caused the molten material to splatter onto the injured employee, who suffered burns to the neck, right side of the face, and right ear.

An employee was directing traffic when a shag truck turned sharply and ran over the employee. They suffered a crush injury to the lower left leg and internal bleeding.

An employee was standing on the street holding a stop/slow paddle and directing the flow of traffic. As they were holding the stop sign, a vehicle attempted to drive around the employee but struck them. The employee sustained fractures to the tibia and fibula and was hospitalized.

A paving crew was working on a roadway when a motor vehicle entered the work zone. Two employees sustained injuries including fractures and damage to internal organs that required hospitalization.

An employee was flagging traffic when they were struck from behind by a car that entered a work zone. The impact of the vehicle threw the employee approximately 45 feet and she landed on another employee. The employee who was struck by the car sustained a head laceration and fractures to the ribs and ankle.

An employee was setting up a worksite on a roadway when they were struck by a vehicle. The employee suffered blunt force trauma to the chest, pelvis, and legs from contact with the vehicle, with subsequent trauma to the neck and head upon falling to the ground.

An employee was on a 12-foot extension ladder while painting the wall and soffit area of a pavilion less than 6 feet off the ground. The employee was using a sprayer when he fell off ladder and landed on his arm on the dirt clay ground. The employee sustained a dislocated elbow and fractures to his right arm and wrist that included an open fracture. The employee was hospitalized and required surgery.

An employee was blasting a water tower. They fell approximately 60 feet off a suspended scaffold and sustained broken vertebrae, a compound fracture of the tibia, a fractured heel, and a rupture to their liver. The employee was hospitalized.

An employee was sandblasting paint from a water tower when the blasting media sprayed across their forearm, resulting in an elbow injury that required hospitalization.

An employee was working from a boom lift, painting wood trim at a second-story roof level. The lift drove over an unmarked dry well, tipped, and catapulted the employee to the ground. The employee sustained fractures to their heels and vertebrae. Fall protection was not being worn at the time of the incident.

On October 2, 2025, an employee was ascending a ladder attached to a scaffold structure approximately 10 feet high. While climbing up, he lost his grip on a rung and fell to the ground, landing on his shoulder. He was hospitalized with fractures to his collarbone and shoulder.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
