An employee was sweeping a floor. The employee fell, landing on the floor and suffering broken ribs and a punctured lung on the right side.

On November 30, 2018, an employee was loading a shaft onto a winding machine using an electric hoist when the shaft caught the corner of the machine and began to fall from the hook. The employee's foot was struck by the falling shaft, partially crushing it and resulting in a big toe amputation. The employee was hospitalized.

On or about November 28, 2016, at approximately 1:40 a.m., an employee was loading a new roll of plastic film on a winder machine when his left hand was pulled into a nip point. His lower left arm was fractured and required hospitalization.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

At 12:45 a.m. on September 21, 2025, an employee was working to clear an obstruction in a machine when his left hand came into contact with a turning screw inside the machine. His index and middle fingers were amputated.

Two employees were changing out a mandrel on a coater converting machine when the injured employee's right index fingertip was pulled by the mandrel, resulting in a partial amputation.

On August 18, 2023, at 10:15 p.m., an employee was chucking a core when their left little finger got caught between the chuck and the core, resulting in amputation.

On August 10, 2023, at approximately 6:45 PM, a maintenance employee was replacing a cover on an electric motor when the motor assembly dropped to the floor, crushing the employee's right little finger and amputating the fingertip.

An employee was performing maintenance on a sheeter. He had pried up a steel access plate and was lifting it when it slipped and fell on his left middle finger. The finger was pinched against the floor, and part of the fingernail and fingertip was amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
