On 5-20-2019, at approximately 11:30 AM, an employee was attempting to stand up in the bathroom when he slipped on water and hit his head against the sink. He suffered blunt force trauma to the head.

An employee was running a moulder machine when it jammed. The employee turned off the equipment, opened it, and was trying to clear the jam when the machine amputated a finger. The moulder machine had not completely stopped and was not locked out/tagged out at the time of the incident.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee was under a conveyor working to repair a drive belt. His arm was pulled into the conveyor and broken. He was hospitalized.

An employee fell to an office floor, landing on her right side and suffering a broken hip, a broken right arm, and a concussion. She was hospitalized, requiring surgery.

An employee was clearing a small piece of wood that had jammed in the back of a machine. A cutting blade in the machine lacerated the employee's right forearm. The employee was hospitalized, requiring surgery.

An employee was operating a stand-up forklift to travel from the warehouse to the breakroom area. The forklift struck the breakroom wall, causing a fracture/crush injury to his left foot.

An employee was cutting a piece of wood with a table saw. The wood bound in the saw, which then pulled the employee's right hand into the blade. His middle finger was cut and fractured.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
