Final narrative
An employee was removing a stack of metal blank material from the blank stacker on the press. The riser that the blanks sit on had moved off the rollers while the blanks were stacking, therefore, the material would not roll freely. The employee attempted to move the blank material down by hand. The original stack fell striking his left ring finger resulting in a fingertip amputation.
An employee was operating a stationary projection welder when a nut misfed onto the part. The employee went to move the misfed nut. The ram on the welder actuated and the employee's left index finger was pinched between the ram and the electrode. The finger was amputated to the first knuckle.
An employee was using a remote-operated overhead crane to pick up a stamping die. The crane cables caught the cleat of a die as the employee was moving the crane to pick up another die. The die fell and crushed the employee's right foot.
