An employee was attempting to lift a patient off the ground. The patient tried to help and pushed himself back onto the employee. The employee fell over the curb and the patient landed on top of him, resulting in knee injuries that required hospitalization.

An employee was moving a patient off one bed and onto another using a draw sheet to assist the patient with a "log roll." The patient rolled off the bed and onto the employee, pinning the employee's leg underneath. They sustained a right lower leg fracture.

An employee stood in front of a patient with their hands under the patient's arms in order to lift them. During the lift, the patient pulled their feet up and grabbed the employee's neck causing injury to the employee's T1 and T2 vertebrae.

An employee was helping a patient sit up. The patient grabbed the employee by the neck, and the employee suffered a herniated disk with nerve compression. The employee was hospitalized.

An employee was helping a patient get out of bed when the patient fell on top of him. The employee sustained a back injury.

An employee exited the ambulance van she had been driving and went to lower the hydraulic wheelchair ramp/lift on the side of the vehicle. The lift would not lower into position. She went around to enter the van to continue troubleshooting the lift when the van began to roll backward as the employee was partially out of the driver s side door. She reached in to stop the vehicle but the transmission shifted into reverse. The van turned and backed across a residential street, dragging the employee approximately 15 feet until it went up on a curb and the employee s left leg was pulled under the left front tire. She was hospitalized with fractures to her right wrist, right hand, and left fibula.

An employee was responding to an emergency call at the end of a pier when they stepped into a gap in the pier boards, resulting in a broken tibia, fibula, and ankle.

On February 19, 2025, an employee was stepping off a 4-foot metal table when he lost footing and fell to the concrete shop floor, resulting in a hip injury.

An employee was getting into the ambulance after responding to a call, slipped on the wet running board and fell to the ground. The employee struck their head and sustained a concussion.

An employee was lifting a patient weighing approximately 400 lbs. and fractured their L5 vertebra, requiring hospitalization.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
