Final narrative
An employee was cleaning a cooling coil on a rooftop HVAC unit. He removed a panel to the unit to wash the coil from the inside out. His left hand was inside the unit when the fan blade struck and amputated his left index finger and lacerated his middle finger.

An employee was handling sheet metal and assembling ductwork in the shop. While handling sheet metal, the employee lost his balance and fell, striking his hip against the concrete floor. He had a tape measure in his pocket on the hip on which he fell. Ductwork that was being handled then fell on the employee. The employee was hospitalized with a broken hip.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.
