Final narrative
A temporary employee was operating a horizontal bandsaw. The employee started to adjust the in-feed rail on the saw using a wrench while the saw was running. The wrench slipped in the employee's hand and his right index finger contacted the saw blade. The employee was taken to the hospital and the index finger was amputated in the hospital the day after the incident.
An employee was using a foot-operated saw. The employee's gloved right hand was pulled into the saw area, and the employee suffered an amputation to the thumb tip.
A forklift operator was traveling down a ramp with a stack of pallets, while another employee was walking up the ramp. The employees collided and the injured employee was struck by the pallets, requiring hospitalization for a skull fracture.
