An employee was unloading rail cars and came in contact with the indexer. The employee's right foot was caught between the indexer and a rail car causing a laceration to the top of his foot and two fractured toes.

While installing a belt driven motor assembly an employee's finger was caught between a belt and the sheave resulting in a laceration to the little finger on his right hand. The employee was admitted to the hospital in preparation for surgery.

An employee was investigating the discharge of a rotary airlock. He removed the inspection door, visually inspected the spout, and thought he saw a suspicious buildup. When he attempted to remove the buildup inside the spout with his left hand, his glove was caught by the running airlock, which then pulled his hand in and amputated three fingertips.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was testing a limit switch on a bale bagger. The bagger was activated and moved forward about 9 inches, pinning the employee's right hand. He was hospitalized with a broken thumb and a laceration to the hand.

An employee was operating a cyclone machine and noticed cotton hanging under the machine. As the employee went to remove it, his right thumb contacted moving parts. The employee sustained an amputation of the right distal phalanx of the thumb.

Two employees were working on a 600-amp breaker. One employee suffered an electrical shock and burns to his face, head, upper torso, and his left hand due to an arc flash.
