An employee was carrying a mirror into an apartment building through an interior staircase. As the employee was entering the doorway, the mirror inadvertently hit the doorframe. This caused the mirror to break. A portion of the mirror fell and lacerated the employee's left forearm, severing arteries and cutting two nerves.

An employee was loading a dish washing machine. A plate bumped against the machine and broke, lacerating the employee's left arm. The employee was hospitalized and required surgery.

An employee was throwing a mirror into a dumpster when the mirror shattered and lacerated the employee's arm.

Rycon Construction, Inc.

Two Rycon employees were installing glass marker boards in a nearly completed space. Each employee had a suction cup on the face of the board and a hand on the bottom of the board. Upon hanging the third board in sequence on a hang strip mounted to the wall, the board needed to be moved about half an inch to butt tightly to the adjacent one. When the employees moved the marker board, it shattered causing half of the board to fall and cause a severe laceration to an employee's left wrist.

On May 15, 2023, an employee removed a mirror from the bathroom to discard it. As the employee went to place it on the cart, the mirror broke. The employee sustained a laceration on the right forearm that required surgery.

FCC Environmental Services Florida, LLC

An employee was lifting a round, glass table top while working on a waste collection route. The glass table top shattered and lacerated the employee's arm.

An employee was using a metal shear when it amputated the tip of his right index finger.

An employee was spotting a suspended forklift load and went to rotate the load when they were struck by the left tire of the forklift, resulting in a broken foot and lower leg.

An employee was unloading glass panels from a delivery truck and riding the lift gate down with a cart of the panels. The material shifted when the lift gate reached the ground and the glass panels fell onto his right foot. The employee sustained three fractures to his foot.

On March 26, 2025, an employee was operating a forklift. They were passing a stationary skid steer when their left foot contacted the skid steer, resulting in a crushed left ankle. The employee was hospitalized and required surgery.

An employee was using a table saw to cut a splicer piece when the metal kicked back and the employee's finger contacted the saw blade. The employee sustained a finger amputation.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

Haven Steel Products Inc

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Vestas-American Wind Tech

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

Alliance Ag and Grain LLC

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
