Rolling Frito-Lay Sales, LP

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.

An employee was exiting a trailer for it to be loaded when their right ankle was pinned between a piece of angle iron that was anchored to the floor and a long pallet being pushed by a powered industrial truck (PIT). The employee sustained an ankle fracture and was hospitalized.

After completing a run with an oil distributor truck, an employee was working to return (suction) the hot oil to the tank of the truck. When the employee opened one of the valves, hot oil (approximately 385 degrees) sprayed their face, resulting in first-, second-, and third-degree burns.

An employee was cleaning the cylinder on a 55-gallon barrel pump. While he was cleaning built-up material from under the baseplate, the piston started moving and caught his thumb against the seat. Three-quarters of an inch of the thumb was degloved; he was hospitalized, requiring surgery.

Great Southwestern Construction

An employee was actively involved in the rigging process and was providing verbal directions to both the forklift operator and the spotter. The operation was temporarily halted after the pole section was raised approximately two to three feet out of the ground to adjust the rigging. The employee walked under the forks and gave direction to lower them. While doing so, the employee placed their right hand on top of the rigged pole for support and used their left hand to adjust the rigging components. The base of the mast came in contact with the employee s right hand, resulting in a laceration and amputation of the right index finger.

A FESCO employee was on top of a trailer assisting a forklift operator with placing equipment onto the semi-truck trailer. While on top of the trailer, the employee stepped backward and fell off the trailer to the ground, landing on his back. The employee was hospitalized for a fracture of their C5 vertebra.

An employee was walking through a parking lot after leaving a shop when he slipped on ice and fell to the ground, resulting in a fractured hip.

BAUER CONSTRUCTION, INC.

An employee was installing PEX (cross-linked polyethylene) piping material for a propane line connection. He was cutting off excess PEX piping material with a PEX cutter. He noticed an unassociated piece of material was in the way of the PEX cutter. He reached up to move the infringing material, and his arm came down on the blade of the PEX cutter resulting in a severe laceration to his arm.

Bighorn Canyon National Recreation Area

An employee was offroad, looking for a lost hiker. The employee fell off a canyon rim edge and landed on rimrock 5 or 6 feet below, suffering a separated shoulder and a head injury that required stitches. The employee was hospitalized.

An employee had been assisting fire crews with an active wildfire. The employee suffered a heart attack and was hospitalized for surgery.
