An employee was using a propane welding machine, set at ground level, while standing in a scissor lift inside an empty room under construction when the employee experienced headaches, dizziness, nausea, and fatigue due to carbon monoxide exposure. At about 8:30 a.m. on August 23, 2022, an Icon Mechanical Construction and Engineering employee was hemming a piece of sheet metal with a metal brake. After making the initial bend, he slid the bent end of the metal into the brake. The brake then closed on his right thumb, amputating the tip.
Three employees had just retrieved a wooden beam from under a section of ductwork. One of them was bending over to pick it up when the duct section shifted and a piece of angle iron slid off it onto that employee's left thumb. The thumb was broken and the employee was hospitalized, undergoing surgery.
An employee was passing through a building when she suffered an allergic reaction to a cinnamon air freshener. The employee began to feel dizzy and ill, ultimately becoming unresponsive. The employee was hospitalized.
