An employee was working outside assembling equipment in heat and humidity. The employee sustained heat-Illness/dehydration.

An employee was using a large pot to carry hot oil from the fryers. His left foot got caught on a hose that was on the floor and the pot fell. When the pot hit the floor, the hot oil splashed upward onto the employee's face, hands, and legs causing second-degree burns.

An employee was walking from their work station when he stepped on a wooden board and rolled his ankle. He sustained a pelvic injury that required surgery.

While on a scissor lift, an employee was cleaning an overhead fan. The employee tripped over the hose of a wet/dry vacuum and his right hand struck the running fan blade resulting in amputation of the middle fingertip. The fan was not guarded at the time of the incident.

At 3:23 p.m. on March 21, 2022, an employee was returning to her workstation when she tripped over a plastic band with her left foot, suffered a broken right femur as she tried to regain her balance, and collapsed to the floor. She was hospitalized.

An employee was climbing down a track ladder. The employee's left foot twisted on a step as their other foot reached the floor, and the employee's shin was broken.

An employee was working as a background extra for a movie. She slipped while walking through the holding/catering area, falling to the ground and striking her right elbow. The employee was hospitalized with a fractured elbow.

An employee was dismantling a film set. He was standing on top of a wooden beam when he lost his balance and fell approximately 10 feet to the ground below, striking his head on a standing light. He sustained injuries to his head, torso, and shoulders. A guardrail was in place at the time of the injury.

An employee had been sitting in a truck that was not on and had no air conditioning. The employee felt ill and was hospitalized for heat illness.

A photographer was walking through a studio toward the production table during a photoshoot. The photographer tripped on a power cord that was taped in place on the floor with a cord cover. The photographer fell, fractured her right hip, and was hospitalized.

An employee was on a temporary loading dock, receiving pieces of a large tent structure from a telehandler. She was working to straighten a packing blanket when the telehandler lowered it's tines onto her left hand. Her middle finger was degloved from the first knuckle up. The finger required surgical amputation from the first knuckle up.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
