An employee was removing a sticker shell (piece of steel) from a mandrel bar. The employee used a torch to burn off a section of the sticker and a crane removed that section from the bar. The employee then began to weld a washer to the remaining section of steel on the mandrel bar. The sticker fell off the mandrel and broke into three pieces, with one striking the employee when it fell to the ground. The employee sustained fractures to the right foot and ankle.
