An employee tripped and fell while walking to a car in the parking lot, breaking the right arm.
A forklift was lifting a crate so that parts could be put up about 13 feet above the ground. An employee was in the crate, on the far side away from the boom, reaching to put up a part. The crate came off the forks and fell to the ground. The employee suffered an injured wrist.
A parts delivery driver offloaded two boxes from a van onto a hand truck. As he was pulling the hand truck back, he slipped and fell, his right side hitting the ground. He suffered a broken right hip and was hospitalized.
