The blade was being kept in place by a 4-by-4-inch piece of wood while he pulled a lever to force the blade flush against the block to secure it for the inspection. The employee sustained an amputation to the left thumb tip.

While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

A temporary employee was walking to the wood pile area when they were struck by a wheel loader, resulting in a fractured tibia and fibula.
