A temporary employee was installing electrical wire from a ladder when they missed a rung and fell 1-2 feet to the floor. The employee's elbow and knee were injured.

An employee was setting a column on a footing. A choker was around the column and forklift being used. The column began to slide, and the employee's right hand was caught between the choker and the column. The employee's middle and ring fingers were amputated at the middle knuckles.

An employee was cutting material with a circular saw. The saw cut the tips of the employee's fingers, causing an amputation and injuries requiring stitches.

A temporary employee slipped on ice and fell to the ground, suffering a leg injury.

An employee had placed a casting on a crane hook and was operating the crane by remote control. When the employee grabbed the casting to turn it, it fell off the hook and struck the employee's fingers. The employee suffered a partial amputation to the right ring finger, as well as lacerations to the right ring and middle fingers.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee was putting up decorations when she slipped off her desk and fell to the floor. She was hospitalized for three broken ribs.

An employee was cutting a ductwork hanger. The employee finished cutting one end of the duct and came down from the ladder. The employee was walking underneath the duct when the ductwork fell on them. The employee sustained a broken rib and required surgery for a vertebra.

A temporary employee was in the locker room when they fell and sustained a hip fracture.

An employee was holding the door for contractors who were leaving the property when his little finger was caught in the door. Half an inch of the employee's left little finger was amputated.

An employee tripped and fell to an operating room floor, suffering a broken hip.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
