An employee was cleaning the roller on a printing press when the employee's right thumb was pulled into the in-going nip point, partially amputating the thumb tip. The press was unguarded at the time.

An employee was working with a pallet bagger, trimming excess corrugate off a tray. The load was raised on a lift table. The scrap being trimmed activated the lift table's photo eye, causing the table to lower. Its frame pinched the steel toe of the employee's left boot between a bracket and the concrete. The employee suffered fractures in the left foot and was hospitalized.

An employee was completing a quality check at the coating section of the paper machine. As the employee was clearing a coating buildup from the applicator roll, the employee's left hand was caught in a nip point between the applicator roll and coater roll and crushed.

While an employee was repairing a water condensate motor and pump, the grating covering an adjacent u-drain dislodged. This caused the employee's right leg to drop into hot water condensate resulting in a thermal burn to the lower right leg.

An employee was pushing a loaded pallet to the end of a conveyor at a press. The employee noticed a plastic pad lying in front of the pallet and was removing it when the pallet, still rolling, pinched the employee's right ring finger against the metal conveyor end stop amputating it.

On 10/10/2018, around 8:30 a.m., an employee was attempting to replace a motor on a press machine when the employee was pinned between the motor and the machine frame, suffering contusions that required hospitalization.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was resetting a slitter machine's counter when their left hand was crushed under two rotating score shafts. The employee sustained amputations to three fingers on their left hand.

An employee went to retrieve dunnage for a co-worker when his right hand became caught in the rollers on the box transfer conveyor. The employee sustained a partial amputation to his middle fingertip.

An employee was working in the converting area installing a glue tote, and became ill from the heat. The employee sustained heat exhaustion.

An employee was troubleshooting a mini-feeder. As the employee was tapping the rotating drive chain with an Allen wrench, the employee's right hand came into contact with the chain. The employee suffered partial amputations, lacerations, and fractures to the middle and ring fingers. The machine was unguarded at the time of the incident.

An employee was between two sections of a printer when they sustained amputations to three fingertips on their right hand from the running print rolls.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Asher Enterprises, Inc.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
