An employee was wiping debris from the bottom of a bundle of lumber when the bundle shifted and pinched the employee's left middle finger, amputating the distal tip.
The injured employee was assisting a forklift operator in the preparation a load of solid core doors for a customer. The injured employee was standing on the platform where the doors were placed for transport and the forklift operator began to move the load. The load shifted and the employee was struck and caught between the solid core doors. The employee sustained a fractured rib and pneumothorax.
