An employee was loading an empty trailer with rolls of paper using a forklift. The employee began experiencing irritation to his eyes, lips and respiratory system. The employee started having chest pains and leg muscle spasms approximately two hours later and was hospitalized.

An employee was sticking a rod through an inspection port to clean lime from the bucket elevator inside a lime kiln. He slipped and the rod and hot lime came out of the inspection port. The solid lime particles contacted his right forearm, causing second degree burns that required hospitalization.
An employee was attempting to close a valve while standing on a platform. One of his feet slipped off the platform, causing him to fall. He was burned on his left lower leg when it came into contact with the condensate that had been heated with steam.
