An employee was bringing up mail in a cart from the basement. The employee exited the freight elevator and went to open the door to the lobby with her left hand. When she pulled on the door it gave no resistance and flew open. The employee's left pinky finger was caught and fractured between the steel casing for the doorstop and the door handle.

He was standing on the first level of the racking system when his foot slipped and he fell 4 feet to the ground landing on his right side. He was hospitalized with fractures to his right collar bone and several ribs on the right side. The employee also struck his head on the ground, scraped his right knee, and his left middle and ring fingers were cut by a nylon band that he grabbed as he fell. The cuts required stitches.
