An employee was moving a box of pillows when he tripped over a piece of tape that was hanging off the box. He fell to the floor, fracturing three ribs and spraining his left shoulder and neck.

An employee was walking across the top of an open dumpster trying to level out the trash in it. He fell from the top metal ledge of the dumpster and suffered a broken left ankle that required surgery.
