An employee and a coworker were moving a loader and a coupler, which was on a pallet. They began prepping the coupler to be attached to a hoist's boom. They partially attached the coupler to the boom when the sling broke, causing the coupler to fall down and strike the injured employee's left shin and foot. He suffered metatarsal fractures and tarsometatarsal dislocations.

Two employees were performing maintenance activities on a bulldozer. They were using a track hammer and sledge hammer to drive the master pin in when a metal fragment from the sledge hammer broke off and struck the employee in the neck, resulting in hospitalization.
An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.
