Paccar Engine Company

An employee had removed a platform grate to expose the equipment below and replace a broken shaft. The employee slipped and fell backward through the open grate, falling approximately 12 feet to the floor. They sustained fractures to their skull, T1 vertebra, right shoulder, and rib, as well as a torn anterior cruciate ligament (ACL).

An employee was pushing an engine block onto a conveyor when their right thumb was pinched between the engine block and a clamp inside a machine. The employee sustained a laceration and an open fracture to the thumb.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

DMAX, LTD.

An employee was moving a hoist to the end of a bridge rail to create space for another hoist on the same rail. The hoist being moved, along with its supporting beam, detached from the bridge rail and fell, striking the employee in the head. He suffered a contusion and laceration to the head and was hospitalized.

An employee was operating a machine. When they performed a motion to push a machined engine block away from their body, they experienced a back strain, resulting in hospitalization.

On September 13, 2025, an employee had been working at an outdoor event. At the end the day, she was walking around outside the facility to make sure everything was put away and felt overheated. The employee was hospitalized for heat-related illness.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
