Final narrative
An employee was dumping a chip hopper. His fingers were between the hopper's box tubing, where the forklift forks pick up the hopper. When he released the dump latch to dump his chips, the weight of the hopper rolling forward caused the box tubing to slam against the forks. This created a pinch point that smashed and amputated his right index and middle fingertips.
