An employee was rigging down a derrick. The board he was standing on shifted, causing him to lose his balance and fall to the ground about 8 feet below. He suffered a broken left ankle.

An employee was putting a collar clamp on 8-inch drill collars in a mouse hole. His right index finger was caught between the collar clamp and a shop hammer (used to tighten collar clamp). The fingertip was amputated and required surgery.
