An employee was using a powered pallet jack (ride on the back style) to pick up orders. The injured employee was walking in front of the PIT, looking backwards, and ran into a stationary stack of materials. The employee operating the PIT was unable to stop the PIT before it struck the injured employee pinning him between the PIT and the stationary materials and breaking the employee's left leg.

An employee had just come out of the restroom and was waiting by a stack of pallets. A coworker turned on his pallet jack and moved the handle, causing it to move in the wrong direction. The jack then hit the employee's left leg, breaking it above the ankle and requiring surgery.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

An employee was working from the elevated bucket of an excavator when they fell between two tanks and landed on the ground. The employee sustained a fractured leg.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
