Final narrative
The injured employee and two co-workers were approached to assist a truck driver lift a small scissor lift back onto its wheels after it tipped over. While lifting the scissor lift, the injured employee's left index and middle fingers were caught in a pinch point when the machine was set upright resulting in amputation of the left index finger and a severe laceration to the middle finger.
