An employee was driving a powered industrial vehicle (PIV) into a pump meat cooler to retrieve a vat of product. As he maneuvered into position, the vehicle slipped on the wet floor and the employee was pulled out of the PIV. He was pinned between the PIV and a vat of product, suffering rib, vertebrae, and pelvic fractures.
