An employee was working on a palletizer machine when it jammed. The employee was trying to clear the jam when the machine caught their left middle finger. The fingertip was amputated halfway up the nail.

An employee was manually rotating a 4-foot-wide grater drum (without its drive chain and motor attached) to do maintenance. The employee's right little finger was pinched between the bar and another part, causing an amputation to the finger.

An employee was cleaning a bean-grading machine in preparation for sanitation. The employee was opening the doors to clean the machine with a power washer when his right ring finger was caught by a roller with a solid bar. The finger was amputated at the second knuckle.

An employee was working with an automatic can palletizer when he noticed cans had fallen off a pallet. While the employee was resetting the cans, he was struck in the right leg by the machine causing a fracture to the tibia and fibula.

On August 31, 2018, an employee was clearing debris from a small piece remover machine. The employee suffered a partial amputation to the left index finger, a full amputation to the left middle finger, and an injury to the left ring finger. The machine was guarded at the time of the incident.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was adjusting the tension on the fruit distribution conveyor belt. While testing the tension, the employee's left hand/arm was pulled into the rollers. The employee's lower left arm was fractured.

On September 22, 2025, an employee slipped on a wet surface and fell to the ground. He suffered a broken right hip and was hospitalized.

Forklift drivers were unloading product from a semi trailer. One employee stopped their forklift about 15-20 feet from the back of the trailer, got out, and began adjusting the forklift's forks. Another forklift backed into the employee, who suffered a compound fracture of the left tibia and was hospitalized.

An employee was in the janitorial closet rearranging the janitor's cart. She stepped back and tripped over the wheel of the cart, causing her to fall to the ground and sustain a fractured hip.

An employee was stacking totes on a pallet jack when their right little finger got caught between totes and the fingertip was amputated.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
