On 9/29/2017, an employee was spraying insulation to the interior walls of an attic. The employee was standing on a 2 x 6 plank, which had been set on the guardrails of a floor opening, when the plank cracked and the employee fell approximately 14 feet to the lower level. The employee sustained a head contusion and fractured vertebrae.
An employee was installing a gutter on a home and fell from an extension ladder. Upon striking the ground, the employee suffered a chin laceration and possible fracture to the left ankle.
