Final narrative
An employee was fabricating a pipe using normal hand tools and a crane when a flange fell on his right foot, fracturing metatarsals 1-4. He was hospitalized for surgery.
An employee was assisting a co-worker empty 55-gallon trash barrels into a self-dumping hopper on a forklift. As the injured employee reached to help empty the drum, the co-worker used the forklift to move the barrel. The drum shifted suddenly and pinched the injured employee's left thumb between the barrel and the hopper, resulting in a left thumb fracture at the first joint.
An employee was rolling a piece of carbon steel pipe from a conveyor onto a storage rack. The pipe was 14 inches in diameter, approximately 65 inches long and weighed about 300 pounds. When the pipe shifted and began to fall between the supports of the storage rack, the employee attempted to regain control of the falling pipe. The employee s fingers were caught between the pipe and the storage rack resulting in lacerations to the middle and ring fingers of the employee s left hand.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
