An employee was adjusting the forks on a skid steer when his right index finger got pinched between the forks and the frame of the skid steer. The finger was crushed and required partial amputation of the fingernail area.

He went to get the rock out and the tailgate came down, crushing his right middle fingertip. The employee sustained a fracture of the distal phalanx and required surgery.
