An employee was loading cargo flat racks onto a barge when his fingers entered the 1-inch gap between the racks and a spreader bar. The flat rack came to rest on the barge, and the spreader bar continued downward and crushed his pointer and middle fingers. His fingers were later medically amputated.

JONES STEVEDORING COMPANY

Employees were unloading logs on a logging vessel. The injured employee was standing on the deck as a load came in. The sling popped off the load and the end of the choker struck the employee's lower left leg, resulting in a shin fracture.

An employee was exiting a confined space while carrying a pressure gun when the trigger activated. Water penetrated the employee's left arm, resulting in injury.

The employee was putting shackles on the grab of a vessel-operated crane. As the crane was lifting the grab, it started to swing toward the hopper. The employee reached to stop it when they were caught and lifted by the grab and pinned between it and the hopper. The employee sustained a pelvic fracture and lacerations to the arm and pelvis.

U.S. Army Corps of Engineers, Northwestern Division

An employee was launching a boat during rough sea conditions. The employee lost balance, causing their left index and middle fingertips to be caught in the eye of the bow line, resulting in amputation.

U.S. Dept. of Navy

Two employees were in a boat pulling a 45-pound anchor out of the water by the rope. The rope tightened and as the employees were attempting to create slack in the rope, the injured employee began to slip. His hand was caught between the rope and the inside of the platform boat, resulting in amputation of the right middle finger with bone loss.

An employee was securing a power cord to a refrigerated truck container. A truck started backing up to connect with the container and the employee was pinned between the tires of the container and the bolster of the truck, resulting in a fractured leg.

An employee was exiting a tank barge's cargo tank after pressure-washing its inside. He was resting at the top of the fixed ladder, with his upper body out of the tank hatch, when he collapsed and fell about 13 feet into the cargo tank. The employee sustained a fractured spine requiring surgery. The employee hospitalized.

An employee was using a pocket knife to cut a zip tie that was holding the slacked portion of line on a ratchet strap. The ratchet strap opened unexpectedly, causing the employee to lacerate an artery in his left forearm. The employee was hospitalized and required surgery.

An employee was walking across a catwalk on a vessel. The employee slipped and fell, striking their right side on the catwalk and suffered broken ribs on that side. The employee was hospitalized.

An employee was spray painting an area of a marine barge using a pressurized paint gun. Her glove became stuck to the gun. Her left hand came out of the glove and while trying to adjust the gun, paint was injected into her left middle finger.

AAA Complete Building Services, Inc.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

U.S. Postal Service

An employee was walking back to a carrier case with mail in her hand when she tripped over a tub. Her back overarched as she fell, resulting in a fractured back that required hospitalization.

Virginia International Gateway Terminals, LLC

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

An employee was washing an aircraft from a scissor lift when they fell to the floor. The employee sustained a back injury and lacerations to their head and neck.
