An employee was pressing a part in a brake press machine when his finger became caught between the piece and the machine. The right index finger was partially amputated at the middle knuckle; it was later surgically re-attached. He also sustained a middle finger fracture and crush injury, and a laceration to the ring finger. The employee was hospitalized.

An employee and a co-worker were replacing an air filter in a dust collector using a scissor lift. The co-worker pushed the filter with his foot so that the employee on the ground could grab the filter when the employee's finger was smashed between the filter and the scissor lift, resulting in an avulsion and an amputation of the right ring fingertip.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

On September 16, 2025, an employee went to pull a weld stand when he tripped and fell, landing on the floor on his right hip. The employee suffered hip and fibula fractures that required hospitalization and surgery.

An employee was accessing a tensioned hook that required a hoist mechanism to be engaged. He was unhooking the lift hook from a shelf column when the "up" button was pressed, which released the tension on the pneumatic air hoist. The employee's right index finger was caught between the hook and the column, resulting in a soft tissue amputation and fracture to the fingertip.

A crane was lifting a pole out of a zinc bath in the galvanizing department when the chain attached to the pole broke. The pole fell back into the bath and liquid zinc splashed onto an employee, who suffered third-degree burns to an arm, the abdomen, the torso, the upper left side of the chest, an ear, the neck, and a hand.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
