Final narrative
An employee was attempting to install a ship's ladder into a ceiling opening. He was leaning into the hole from the roof and using a cable come-a-long to hoist the ladder up to the opening. The hoist then slipped, causing the ladder to fall and pull the employee down 10 feet through the opening. He landed on the ladder, breaking his left leg and seven ribs.
Employees had cut out the concrete slab foundation inside a building that was being remodeled and were placing a sewer line in a 2' deep trench. The injured employee was walking towards the exit when the roof of the structure collapsed, striking and injuring the employee.
