Final narrative
While installing a waterline connection, an aluminum trench box weighing 400 lbs. was removed so the legs could be adjusted. A mini excavator was used to tilt and hold the box while the leg pins were removed. The excavator operator set the box back down on all four legs and the legs pushed up inside the box causing it to fall over and strike one of the employees on his lower body resulting in a broken pelvis.

An employee was assisting with assembling a trench box. A forklift was used to carry a 150-lb. steel spreader tube which was to be placed between the trench box sheets. The forklift tipped forward, causing the spreader tube to roll off the forks. The tube struck the ground and pivoted in a "baseball bat swing" movement, striking the employee on the left side of his torso below the armpit and also on the outside of his lower left leg. The employee sustained a broken rib, internal bleeding, bruising, and contusions.

The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.
