An employee had arrived at a patient's home and was preparing to deliver oxygen and durable medical equipment to the patient when the employee slipped on ice or snow and impacted the ground, fracturing the right tibia and fibula.

On January 1, 2018, an employee slipped and fell on ice in the parking lot, suffering memory loss, a headache, and a suspected minor brain bleed.

An employee was walking toward the company vehicle when he noticed that there was oxygen gas venting on the side of the van. He opened the tri-cock valve to bleed the pressure down from 120 PSI, opened the doors to vent the van for about 5 minutes, and then closed the doors. As he walked across the parking lot, he lit a cigarette lighter, which caused a flame to ignite on his left shirt sleeve. He suffered second and third degree burns to his chest and underarms, requiring hospitalization.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

The school bus driver honked the horn, signaling that the patient was ready to be assisted off the bus. The employee stood up and then tripped and fell over a rug and possibly the edge of an ottoman. The employee was hospitalized with a fractured right humerus at the shoulder.

An employee was assisting a client when the client fell on top of the employee. The employee's hip was fractured.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
