An employee was using a lifter to lift a rack of steel sheets when the lifter caught the edge of a hot block. The hot block then kicked toward the employee's left hand, pinching it between the hot block and lifter body. The employee's ring fingertip was partially amputated.

Three pieces of metal, weighing approximately 250 pounds each, were being lifted using slings attached to an overhead crane. An employee was guiding the metal as it was lifted out of a rack. The metal pinched the employee s finger against the rack resulting in the amputation of his right ring fingertip.

An employee was opening vertical coil tongs that were on the floor. As the employee began to open the device, it fell over and struck him on the right side. The employee suffered a right leg fracture and three broken ribs.

An employee was trying to clean built-up product from the sensor/photo eye of the conveyor on a weigh blend system, which was failing to start after being calibrated. The tray of the conveyor actuated, and the tray's reciprocating motion caught the employee's arm, fracturing the left humerus. The machine was not locked out at the time of the incident.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Stalworth Underground LLC

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

KEN FORGING, INC.

Two employees were loading a steel-forging die into a forging hammer. One employee was pushing the die into place on one end of the hammer with a forklift. The injured employee was on the opposite side of the hammer making adjustments to level the die. When the forklift pushed the die into place, the injured employee's right middle finger was pinched between the stationary leveler and the die., resulting in a partial amputation to the fingertip.

An employee was grinding a part on a belt grinder when the part slipped and his left thumb was caught between the belt and the guard. The employee sustained an amputation involving loss of bone from the thumb tip.

An employee was operating an overhead crane that was carrying a die set. The hoist pin came out and the steel die set fell onto the employee's left foot resulting in injuries to the foot and toes that required hospitalization and surgery.

An employee was operating a blasting machine. Between parts, the employee was shoveling some steel shot off the floor. While shoveling the steel shot behind the machine, he lost his balance and fell into the chain and sprocket on the back of the blasting machine, resulting in amputation of his right index finger to the first knuckle. The chain and sprocket were not guarded at the time.

An employee walking down a passageway was struck by a loaded fork truck. The employee was hospitalized with broken bones.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
