An employee was installing a plastic strip on a processing line. While he was putting in a lag bolt, it punctured his left hand on the palm side.

An employee opened the hatch of a meat processing machine for maintenance to remove product from underneath the machine. The hatch bumped and smashed the employee's left ring fingertip between the machine and the hatch, resulting in an amputation to the tip of the nail.

An employee was winching cages down to a drop-deck live-haul trailer. The terminal tractor moved in reverse and the employee was pinned between the fifth wheel and the trailer kingpin area. The employee suffered multiple fractures and lacerations.

An employee was using a powered pallet jack to move product. The stabilizer/drive wheel lost contact with the floor. When the wheel caught traction, the employee's lower right leg was pinned between the jack and the wall. The employee suffered a compound fracture to the lower leg.

An employee was moving a tote of chicken when their right thumb and index finger were pinched by the belt drive on the shaker table resulting in a partial amputation.

The injured employee was installing fan belts and alternators. Another employee came into the work area with a torque gun and began poking the injured employee with the tip of the torque gun and then activated the tool. The spinning tip of the torque gun caught the injured employee's left glove and amputated her middle finger.

An employee was working on a hot strip mill when a co-worker approached from behind and pushed in the back of the employee's left knee, causing it to buckle. The employee fell to the ground and sustained torn ligaments in the left knee.

An employee sustained a broken arm while arm wrestling with another employee.

Employee 1 fell on employee 2's leg, causing several fractures in employee 2's leg that required hospitalization.

Two employees were involved in horseplay while walking when one employee fell and struck their head on the concrete floor resulting in a concussion.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 25, 2025, an employee was exiting a yard truck. They slipped and fell approximately 3 feet from the top tread of the steps to the concrete below. The employee sustained a fractured left hip.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
