An employee was backing a forklift out of a racking location when the forklift cage became caught on a post and broke, causing it to move forward. The employee was then pinned between the forklift seat and steering wheel, suffering a fractured tibia and fibula.

The employee entered the battery machine alignment track again to unplug the charging cable. The battery changing equipment proceeded forward and pinned the employee against the forklift. The employee was hospitalized with two fractured ribs.
