A forklift backed into an employee, its rear tire striking her right foot. She fell backward to the ground and suffered a broken left ankle and right foot. She was hospitalized.

An employee was raising an electric ladder to retrieve merchandise from overhead racking. As it rose, the railing of the machine caught his right middle finger against the bottom of the high bay netting supports, causing an amputation just below the fingernail.

An employee was working on a mini-excavator with a stuck ignition. He was shutting down the mini-excavator engine when he suffered an amputation of the left middle fingertip.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

The injured employee was walking through the warehouse. Another associate was driving a forklift with a pallet of large format tile (slabs) when the slabs began to tip. An employee was doing a cutback on a piece of lumber using a chop saw. The saw hit a knot, the piece of lumber buckled, then kicked back and broke the kickback guard. The board was then pulled back into the blade, which lacerated the employee's left index and middle fingers.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was descending a rolling ladder when she fell from the last step and her head struck the floor, resulting in a fractured pelvis.

On September 24, 2025, an employee was using a sledgehammer to straighten the forks on a forklift when a piece of metal broke off the fork and became lodged in the employee's leg. The employee was hospitalized.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
