Two maintenance employees were working together replacing a shaft pin, bushings, and spacers on a crimping machine. While they were attempting to lower the top half via a crane into the bottom half, the parts became misaligned. The first employee used his finger to feel inside the shaft hole and determine why it was not fitting together. The second employee, who was at the controls of the hoist, lifted the part, causing the very tip of the first employee's left index finger to become caught. The first employee suffered a fingertip amputation to the index finger on his left hand.

An employee was patrolling a store using an electric patrol scooter when they lost their balance and sustained an ankle fracture.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.

An employee was standing inside a semi-trailer when the driver of the semi-trailer pulled away, causing the employee to fall backward and strike their head on a steel dock plate. The employee suffered a skull fracture, a fractured L1 vertebra, a broken coccyx, a laceration/abrasion to the head, and a brain bleed.

An employee was riding an ATV, catching up with a stray cow to return it to a pen. The ATV tire struck a hole or other obstacle, causing the employee to be ejected and fall to the ground. His leg was broken and he was hospitalized.

An employee was driving a van in reverse when they fell out of the vehicle and landed on the ground in the parking lot. The employee sustained a fractured left hip.

An employee was backing up a forklift to pick up a pallet of materials when it struck a yellow cement pole in front of a building support beam. His left hand was crushed between the forklift and the pole, resulting in amputation of a finger.

An employee was scraping paint off a building panel while on the ground. Ice fell from the roof and struck the employee. The employee sustained a broken right femur and was hospitalized.

An employee was behind a building loading wood pallets onto a trailer when they were attacked by an unknown individual, falling to the ground and losing consciousness. The employee suffered fractured ribs, contusions, and hearing loss.

An employee was manually loading a pallet jack that was parked on a lift gate when he lost his footing and fell to the ground below, resulting in four fractured ribs.

An employee was removing caulk around a pool at a stadium. Some caulk broke as he was pulling it, causing the knife/scraper in his hand to slip and cut his cornea and eyelid.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
