An employee was attempting to tow a vehicle that had broken down. The vehicle was hooked to the tow truck using a winch. The employee was in the space between his truck and the towed vehicle when the winch controller became jarred and damaged, causing the towed vehicle to lurch forward pinning him between it and the truck and breaking his left femur.

The equipment was not locked out/tagged out at the time.
