An employee was unstrapping a piece of pipe when it rolled off, fell, and hit his foot, fracturing it and requiring hospitalization.
An employee was rotating an A-frame steel weldment on saw horses. As he was laying it down it slipped off the saw horses and his left pinky finger was smashed between the saw horse and the A-frame resulting in amputation of the fingertip (flesh).
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
