An employee was preparing to unhook a boring machine. While standing 5-8 feet away from the machine, a securing bolt broke and the employee was struck by a chain in the head and face, resulting in a blunt force trauma.

An employee was using a string trimmer with a brush cutter attachment. The brush cutter struck a rock and then struck and lacerated the employee's left calf.

An employee was conducting a test on a pipeline. While pushing a "pig" through a 20" pipe using compressed air, the steel door at the end of the pipe was blown off and struck the employee causing a broken arm requiring hospitalization and surgery.

At about 5:00 p.m. on September 15, 2018, an employee was driving a utility vehicle at a natural gas pipeline site when it flipped over. The employee suffered a potential stomach tear and was hospitalized.

An employee was helping a road bore move pipes from one place to another. The employee was strapping up the pipe casing as a backhoe was reversing nearby. The backhoe then struck the stem connected to the pipe's auger, which resulted in a teeter-totter effect, causing the pipe to strike the employee around his right thigh. He suffered a compound fracture to his right tibia and fibula, as well as a twisted ankle.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was acting as a spotter for a forklift operator. While its forks were being raised, the forklift came into contact with a power line. The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

An employee was leaning down to install conduit. When he stood up, a 2-inch all-thread rod caught and lacerated his left eyelid.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was standing on the ground awaiting the lowering of a pipe. After the pipe was lowered to the ground by a skid steer, it rolled and struck the employee's left leg. The employee was hospitalized with a fractured leg.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
