An employee was using a forklift to empty a dumpster into a roll-off container when slack in the lift chain caused the forklift's mast to fall. His left thumb was caught between the mast and the dumpster, resulting in a partial left thumb amputation.
An employee was cutting up a piece of equipment for removal. The employee cut a chain and then threw it over a de-energized sprocket.
