An employee was using a forklift to empty a dumpster into a roll-off container when slack in the lift chain caused the forklift's mast to fall. His left thumb was caught between the mast and the dumpster, resulting in a partial left thumb amputation.

An employee was cutting up a piece of equipment for removal. The employee cut a chain and then threw it over a de-energized sprocket. The counter weight yanked the chain and his fingers into the sprocket assembly, pinching the tips of his fingers. The employee's fingertip was amputated, requiring hospitalization and surgery.

At about 7:15 a.m. on August 28, 2025, an employee was using a construction compactor to stabilize the dirt of a dam that had just been built. The dirt gave way underneath the compactor, which tipped on its left side. The employee fell out and was crushed and pinned between the compactor and the dirt. He was hospitalized, having suffered a broken sternum and broken ribs on both sides, as well as a laceration to the left leg by the ankle joint.

A temporary employee was stripping wire for scrap when the machine crushed their left thumb and little finger.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee.
