An employee and his driving partner were disconnecting a tanker trailer from their tractor. Due to the weather conditions, the release handles on the fifth wheel coupling had ice build-up on them. The employee was disengaging the fifth wheel with a fifth wheel puller when the puller slipped. This caused the employee to lose his balance, falling backward and striking his head on an adjacent trailer. He suffered a head laceration and was hospitalized.
An employee was emptying the skid bin underneath a rotary lock at the bag house, which allows bags to empty polymer wax, when the unit became plugged.
